Site-survey request

Request the measurements and access observations needed to quote a real installation, rather than relying on a photograph of the wall.

What this worksheet records

A site-survey request should explain the decision the survey must support. Identify the site and the particular surface, location or equipment relationship that needs checking. Describe the measurement points and required units without inventing values before the visit. Ask about access, fixing, services and relevant photographs separately. The surveyor should be able to say what remains outside the agreed survey scope rather than being assumed to approve every aspect of installation.

Complete and review it

Agree who can provide access and which person may accept the survey's findings. Use the response_due field to plan follow-up, not to imply an agreed appointment. When findings arrive, retain the measurement method, source photographs where permitted and any limitations caused by inaccessible areas. If installation depends on an unanswered fixing or service question, keep that gate open. A photograph alone should not become a structural, electrical or other safety approval.

Field definitions

  • request_id: A stable identifier for this request, used to connect related rows without merging unrelated records.
  • site_id: A stable identifier for this site, used to connect related rows without merging unrelated records.
  • site_contact: The actual site contact relevant to this row’s declared purpose. Record the source or unresolved question where it is not confirmed.
  • requested_survey_date: The actual requested survey date, with date format or period meaning explicit; distinguish requested, planned, confirmed and observed values.
  • surveyor: The named person or agreed role responsible for surveyor; confirm its real scope rather than assuming authority from a typed name.
  • measurement_location: The actual measurement location relevant to this row’s declared purpose. Record the source or unresolved question where it is not confirmed.
  • required_measurement_mm: The required measurement in millimetres at the stated measurement points; leave unmeasured geometry unknown.
  • substrate_or_surface: The actual substrate or surface relevant to this row’s declared purpose. Record the source or unresolved question where it is not confirmed.
  • access_constraint: The actual access constraint relevant to this row’s declared purpose. Record the source or unresolved question where it is not confirmed.
  • fixing_question: An unresolved attachment or fixing matter requiring the appropriate current instruction and competent response.
  • services_question: An unresolved service or installation interface question, not an inferred technical clearance.
  • photo_permission: The actual scope for taking and sharing site photographs; do not infer public publication rights.
  • owner: The named person or agreed role responsible for owner; confirm its real scope rather than assuming authority from a typed name.
  • response_due: The actual response due, with date format or period meaning explicit; distinguish requested, planned, confirmed and observed values.
  • evidence_reference: The actual attributable source for evidence reference; a reference is not evidence that a decision was authenticated or its content verified.

Prepare a deliberate export

The request can contain named site contacts and sensitive building information. Share it only with the chosen surveyor and relevant site owner. Exclude unnecessary plans or personal details from a public project example; the blank download grants no access to a premises.

Take it to the next job

  • Describe the decision and measurement points before arranging the visit.
  • Record access and media permissions explicitly.
  • Keep inaccessible or untested conditions visible in the response.
